Meridian HealthCare provides employees with the opportunity to purchase Short Term Disability. Premiums increase when moving from one age bracket to another at the beginning of each plan year. Your premiums for this plan are deducted on a post-tax basis.

Benefits Begin	Once paid leave is exhausted but no sooner than 8th day of disability
Weekly Benefit	60% up to $2,000
Maximum Benefit Duration	26 Weeks

Your benefit may be reduced based on other sources of income. Not all disabilities are covered. Please review the plan booklet for important details and limitations pertaining to this plan.
