Meridian HealthCare provides Group Life and AD&D insurance to all eligible employees at no additional cost.

Be sure to assign a beneficiary or living trust to ensure your assets are distributed according to your wishes.

Voluntary Life and AD&D

Meridian HealthCare provides employees with the opportunity to purchase additional Life and AD&D insurance through post-tax payroll deductions. Premiums increase when moving from one age bracket to another at the beginning of each plan year. You may elect to purchase additional life and accidental death and dismemberment coverage in increments of $10,000, not to exceed $500,000.

Evidence of Insurability (EOI):
If you enroll yourself and spouse in this plan when first eligible, you can request up to $150,000 of Voluntary Life/AD&D coverage for yourself and up to $30,000 of coverage for your spouse without evidence of insurability (proof of good health) required. If you do not enroll in this plan when first eligible, any amount of Voluntary Life/AD&D coverage requested later, will require satisfactory evidence of insurability.

Child(ren) $1,000, $2,000, $4,000, $5,000 or $10,000
