Meridian Healthcare has partnered with PeopleOne to offer you a robust wellness program that focuses on your overall health and well- being.

You should complete an annual wellness visit with your primary care physician to catch potential issues early, before they become serious, and to help you focus on wellness and a healthy lifestyle. These visits may include checking your vitals and a physical exam. By completing this, you are eligible to receive rewards, so make sure to get you appointments scheduled each year.

To activate your PeopleOne portal account:

1. Go to portal.peopleonehealth.com/enrollment/meridian
2. Type in your email and click Continue.
3. Fill in your information and click Create Account.
4. Check your email for a message from PeopleOne Health.
5. Click the verification link in your email to create your password and complete account set-up.
